to this form as .sd3 or .cl2 data files

Please Note:
For each potential Canadian or World record, an acceptable record application form shall be completed by Meet Management or by the swimmer and submitted to the Provincial Recorder,ensuring receipt within 15 days of the conclusion of the swim meet.
